How Chronic Stress Disrupts Hormonal Balance & Tips for Regulating Cortisol

In over 20 years of clinical practice, I’m still amazed by all the things I’ve learned outside of school—especially regarding the intricate relationships between stress and hormone balance. One concept that stands out explains why women can experience hormone imbalances during times of chronic stress.

Cortisol is our primary stress hormone, produced by the adrenal glands. The precursor to cortisol is pregnenolone, which is also the precursor to progesterone (often called the “mother” of other sex hormones). When you’re under constant stress, the body prioritizes cortisol production. This shifts pregnenolone away from making sex hormones, setting the stage for hormone imbalance. It’s part of the reason some women skip periods during stressful stretches.

Why Stress Management Matters

Simply put: it’s not physiologically possible to have balanced sex hormones in the presence of chronic stress. No medication or supplement alone can fix this imbalance if the root issue—ongoing high demand for cortisol—isn’t addressed. That’s why developing healthy stress-management strategies is essential.

Cortisol’s Role and Effects

Cortisol regulates our stress response and influences metabolism, inflammation, and the immune system. It’s part of the hypothalamic-pituitary-adrenal-thyroid-gonadal (HPATG) axis, meaning chronic stress can lead to hormone dysregulation.

  • High Cortisol Levels
    Initially triggered by fight-or-flight situations, persistent elevation can contribute to insomnia, mid-section weight gain, blood sugar dysregulation, memory issues, and digestive problems.

  • Low Cortisol Levels
    Over time, constant demand for cortisol can deplete the adrenal glands. The resulting fatigue, foggy thinking, weight gain, and pain often signal that the body can’t keep up with unrelenting stress.

If you experience ongoing fatigue, weight gain, trouble concentrating, or any of the above symptoms, it’s wise to consult a healthcare provider. Chronic cortisol imbalance can exacerbate other health conditions, so understanding your hormone levels and stress response is crucial.

Recommendations for Balancing Cortisol Levels

The cornerstone of managing cortisol lies in healthy stress management:

  • Meditation, prayer, visualization, journaling

  • Yoga, dance, forest bathing, grounding

  • If needed, herbs like ashwagandha, rhodiola, astragalus, holy basil, and lemon balm

Combining these strategies with adequate rest, balanced nutrition, and regular movement helps address the root cause of cortisol imbalance. While medications and supplements can support the process, they can’t solve the ongoing pressure to produce cortisol if chronic stress remains.
